What do I need to bring for my first visit?
Make sure to arrive 15 minutes early to complete all new patient paperwork. Most required paperwork can also be completed on the dental office’s website. At Advanced Dentistry by Design, we are very excited about our website. You are able to gather more information about our office, our doctors, our team, and procedures provided. You can also check out the doctors’ blogs, and of course the Q and A section with me, Terrie! Bring your insurance card with you. The front office will help you navigate your insurance benefits and insurance questions, but we need to have your current card. If you have insurance, you need to be aware of the frequency limitations for your x-rays. For most, the full mouth series (FMX) is current for five years and the bite wings (check-up x-rays) are once a year. If you have current x-rays you will want to complete a release with your new office to have the x-rays transferred prior to your first appointment. Doctors cannot make diagnostic decisions based off a visual exam alone, therefore x-rays are an important part of your first visit. Please make sure to bring a list of your current medications, dosage, and frequency in which you take the medications. Additionally, you will also be asked to complete a HIPAA form to protect your privacy. This will tell us anyone, other than yourself, who we can discuss your dentistry and appointments needed. The first appointment is easy and non-invasive, and a great opportunity to meet the doctor(s) and team to determine if the practice is a good fit for you.
